(a) Emergency Admission in Non-Empanelled Hospital. The onus of informing the OIC Polyclinic within 48 hrs lies with the ECHS member / patient / NOK. Thereafter the OIC will initiate an Emergency Information Report (EIR) after ascertaining the emergency. The bills will be cleared by the patient and submitted to the Polyclinic within 30 days of his discharge from the hospital. Bills will processed subsequently if the EIR is in order and reimbursement will be as per CGHS rates (b) Emergency Admission in an Empanelled Hospital. The onus of informing the OIC Polyclinic with 48 hrs lies with the Empanelled Hospital. However, the ECHS member may also convey the information on his own to ensure initiation of action. Thereafter, the OIC Polyclinic will initiate an emergency referral after verifying the emergency. The OIC will also ensure that the emergency referral reaches the empanelled hospital in time and that the empanelled hospital does not charge patient inadvertently or otherwise. Empanelled hospital bills, once received at the poly clinic along with necessary supporting document, will be processed as per approved rates. (c) Treatment of emergency will be provided by the empanelled hospital, irrespective of the fact whether or not the generalized / specialized service required for treating the patient is covered under the Memorandum of Agreement (MOA) with ECHS. The bills in such cases will be cleared by ECHS as per authorized rates, provided necessary emergency information has been give to the OIC Polyclinic in time by the empanelled hospital. An ECHS member has to submit the following documents to the nearest Station Head at the new location for change of Parent Polyclinic:- (a) An application on plain paper clearly giving the following details such as old ECHS registration number, old address, old parent polyclinic, new address and new parent polyclinic. (b) An undertaking on a separate plain paper stating that ‘ No information has been concealed or suppressed ’. Any false information submitted will make a member liable for termination of his ECHS membership. (c) Proof of new address like electricity / telephone / water bills / RWA certificate etc.. (d) A fresh ECHS application form with relevant columns filled and super scribed ‘CHANGE OF ADDRESS’ and ‘CHANGE OF PARENT CLINIC’. (e) Return the old cards duly defaced.
