Procedure for Processing Family Pension cases of Widowed / Divorced Daughters 2. The following documents / certificates are required for processing the Family Pension cases of widowed / divorced daughters:- (i) Original affidavit sworn before a first class magistrate showing the details of claimant’s marriage including the name of bride, bridegroom and date of death of the groom (widowed daughter) / date of divorce decree absolute (divorced daughter). (ii) Marriage Certificate of the claimant obtained from the Registrar of Marriage showing full details of her marriage. (iii) Original / ink signed copy of Death Certificate of her husband issued by Registrar of Deaths (widowed daughters). (iv) Original / ink signed copy of Divorce Decree obtained from Court of Law (divorced daughters). (v) Certificate of present marital status of claimant obtained from First Class Gazetted Officer / Village Sarpanch. (vi) Income Certificate issued by Revenue Officer / Tehsildar stating monthly and annual income from all sources. (vii) Unemployment Certificate obtained from competent authority. (The case of Family Pension for widowed / divorced daughter can be taken up only after the demise of the pensioner and death / disqualification of other heirs eligible for the same before her.) (a) As on date ECHS has 3.90 lakh members and 9.30 lakh dependants who are authorized for treatment. (b) Reservists who are drawing pension including family pension are also permitted to join the scheme. However, reservists who opted for ex-gratia payment are not eligible. Alert 4. (a) No medical facilities will be provided in Polyclinics or Armed Forces Hospitals for no-ECHS Veterans after 01 Apr 2008. (b) Last day for registration of ECHS membership is 31 Mar 2008.
