Emergency information report of the Poly clinic, should be obtained within 48 Hrs. of admission 4. Smartcard copy. 5. Contingent Bill for the claim amount preferred. 6. Bank mandate form/Blank cancelled cheque. 7. Patient Bill breakup submitted by the hospital that treated. 8. Hospital original Bill 9. Hospital Bill break up
