Page 55 of 56 34. ECHS. Some useful information on ECHS is appended below:- (a) Application forms . The forms are now unnumbered and are self explanatory. These are available in Army/Navy/Air Force Stn HQ. They can also be downloaded from websites: www.indianarmy.nic.in or www.irfcnausena.nic.in or www.indiafoundation.org or www.indianairforce.nic.in. (b) Dependant Add on Cards. With immediate effect, ECHS members are authorized for three Add on dependant cards in-lieu of the earlier two Add on cards. The procedure to get the third card is given below:- (i) Download the application form any of the above websites. (ii) Filled application form with a DD for Rs.90/- in favour of ECHS Regional Centre, Delhi is to be forwarded to the Director ECHS, 6 th floor Chanakya Bhawan, Chanakyapuri, New Delhi-110021. Auth: IHQ MOD (Navy), New Delhi letter DX/900 dated 15 Apr 2008. (c) Status of a widow after remarriage – ECHS membership. The primary criteria for availing the ECHS facility is that the beneficiary must be a pensioner/family pensioner. As the widow continues to be a pensioner even after her remarriage, she can continue to be an ECHS member/become a new ECHS member. (d) Change of status of dependants vis-a-vis ECHS membership. The change in status of dependants caused by marriage, employment, etc is to be immediately informed to the respective Stn HQ. In case it is discovered that a change has been concealed, no matter what the reasons are, as per provisions of para 9 of the affidavit signed by the member, the membership of the ESM and all his dependants will be terminated without any relief. There is no appeal in such cases. An individual cannot be a member of two Govt/PSU health schemes. He/She has to cancel the membership of the other scheme while joining ECHS. One cannot be a member of CGHS and ECHS nor can he/she be a member of the ECHS and a PSU scheme at the same time. If any concealment of facts is discovered, action will be taken as per provisions of para 9 of the affidavit. (e) Reimbursement of medical expenses incurred by ECHS members. With immediate effect, the financial powers of various CFAs are as given below:- Financial Limit Designated Authority Up to Rs. 2 Lakhs Station Commander, Station HQ From Rs.2 Lakhs to 4 Lakhs Regional Director, Regional Centre, ECHS More than Rs. 4 Lakhs Director, Central Organization, ECHS Members desirous of applying for reimbursement of their bonafide medical expenses may submit their claims in prescribed forms to the concerned CFAs.
