The Ex-servicemen Contributory Health scheme provides comprehensive medical, cover to the Ex-servicemen and his family(comprising his spouse dependent parents with income less than Rs 1500/- pm, unemployed sons less than 25 years and unemployed, unmarried daughters)who are in receipt of pension including disability pension and family pension, with no restriction on the type of ailment or the age of the beneficiary. 2. How to join this scheme? Pensioner should visit locations as advertised in National/Regional dailies. For preparation of ECHS Card, he/she should be accompanied by all dependents on any working day after 01 Apr 2003 between 0930h and 1400h with the following documents:- a) Pension Paying Order. b) Certificate of Service IAFY1964. c) Ex-servicemen Identity Card. d) Affidavit as per given format. e) Bank Draft Payable to the Director ECHS as follows:- Monthly Pension Contribution Less than Rs 3000 Rs 1800/- Rs 3001-6000 Rs 4800/- Rs 6001-10000 Rs 8400/- Rs 10001-15000 Rs 12000/- More than Rs 15000 Rs 18000/- 3.
